Terapias de Insulina en UCI

This application calculates the insulin requirements for hospitalized hyperglycemic patients. It proposes two different therapeutic regimens and routes of administration: intravenous insulinization, recommended for unstable critical patients (ICU), and subcutaneous insulin regimen - intensive protocol - for stable patients who can tolerate oral intake or enteral nutrition.

The development of these calculators is based on the algorithm from the NICE-SUGAR clinical trial, which demonstrated a reduction in mortality for ICU patients when glycemic levels were maintained between 140-180 mg/dl. For stable patients, I have adhered to classical recommendations indicating that intensive subcutaneous insulin regimens aimed at achieving preprandial levels between 90-130 mg/dl improve prognosis for both diabetic and non-diabetic patients.

The adjustment of intravenous insulin doses takes into account the trend (upward or downward) observed in the last hour of treatment as well as the amount of insulin responsible for this variation. Bibliography and formulas used for calculation and dose adjustment are attached.
